Tesamorelin is a synthetic peptide built from 44 amino acids and classified with the growth hormone–releasing hormone family. Its sequence corresponds to the human GHRH(1-44) backbone, carrying one structural change at the amino terminus. That change is a trans-3-hexenoyl group placed where the natural peptide would have an unmodified end. The modification is the feature that separates the compound from the endogenous hormone in name, in stability, and in how it is handled in the laboratory.
The hexenoyl cap slows the enzyme step that trims the amino terminus of native GHRH, the same step that shortens its active lifetime in circulation. As a result, the modified peptide persists longer in plasma than the unmodified hormone in side-by-side comparison. Receptor activity stays broadly comparable, because the added group sits away from the residues that contact the binding site. This combination, preserved receptor activity with reduced degradation, explains why the analog was developed instead of the native sequence.
Stimulated growth hormone release leads to hepatic production of insulin-like growth factor 1, a key mediator of many growth hormone effects. In clinical studies, tesamorelin increased IGF-1 levels in a dose-dependent manner, although the response varies among individuals. The drug's effect on visceral fat is thought to involve growth hormone-mediated lipolysis and altered adipocyte metabolism. Muscle mass and lean body mass have also been assessed as secondary outcomes, but changes are generally smaller and less consistent than fat reductions.
Pharmacodynamic studies show that tesamorelin reduces visceral adipose tissue more than subcutaneous adipose tissue in the studied population. This selectivity may relate to differences in blood flow and hormone sensitivity between fat depots. Effects on glucose metabolism and insulin sensitivity have been investigated, with some trials reporting modest changes and others showing stability. The precise relationship between growth hormone exposure, IGF-1 levels, and visceral fat loss remains an active area of analysis.
Tesamorelin binds to growth hormone-releasing hormone receptors on somatotroph cells in the anterior pituitary. Receptor activation increases intracellular cyclic AMP and promotes synthesis and secretion of growth hormone. Because the peptide mimics endogenous GHRH, it amplifies the normal pulsatile release of growth hormone rather than providing exogenous growth hormone directly. This upstream action distinguishes tesamorelin from recombinant growth hormone preparations and from growth hormone secretagogues that act at different receptors.

Clinical study of tesamorelin has centered on adults with HIV-associated lipodystrophy, a condition in which abdominal fat accumulates while peripheral fat is lost. In controlled trials, treated participants showed reductions in visceral adipose tissue measured by imaging, alongside modest shifts in some lipid values. Effects on subcutaneous fat were smaller and less consistent across studies. Whether these changes translate into fewer cardiovascular events remains an open question, because the trials were not designed or powered to answer it.
Tesamorelin is a synthetic peptide that acts as an analog of growth hormone-releasing hormone, a natural hypothalamic signal. Its sequence corresponds to the forty-four amino acid form of the human hormone, with a small acyl group attached near the amino terminus. That modification slows enzymatic breakdown and extends the time the peptide remains active in circulation. The compound was developed as a pharmacological way to raise endogenous growth hormone output rather than supplying the hormone directly.
After injection, the peptide binds receptors on somatotroph cells in the anterior pituitary. Receptor activation raises intracellular cyclic AMP and triggers release of stored growth hormone into the bloodstream. Because the compound works through the body's own regulatory system, growth hormone pulses retain much of their normal feedback control. Repeated administration also raises insulin-like growth factor 1, a hormone produced mainly in the liver. Investigators treat that rise as a marker that the pituitary axis has been engaged.
Because growth hormone is released in pulses, single measurements can misrepresent overall secretion. Investigators sometimes use repeated sampling or overnight profiles to capture the pattern rather than a single value. Provocative testing, in which a stimulus is given and the response is tracked over time, offers another way to characterize the axis. Each approach carries trade-offs between sensitivity, burden on the participant, and the influence of non-target variables.
Insulin-like growth factor 1 is produced largely in the liver in response to growth hormone signaling. Its concentration shifts over days rather than minutes, which makes it practical for tracking changes across a study period. Interpretation still depends on age, nutritional status, and concurrent illness, all of which independently affect the marker. Reference ranges are therefore stratified, and comparisons are usually made within an individual over time rather than against a single population threshold.
Tesamorelin is a synthetic peptide that belongs to the growth hormone-releasing hormone family and contains the same forty-four amino acid sequence as endogenous GHRH, extended at the amino terminus by a trans-3-hexenoyl group. That small fatty acid modification protects the peptide from rapid cleavage by dipeptidyl peptidase-4, the enzyme that shortens the half-life of native GHRH to only a few minutes. Chemically the compound is produced by solid-phase peptide synthesis, purified by chromatography, and supplied as a sterile lyophilized powder for reconstitution.
Regulatory approval in the United States came in 2010, when the Food and Drug Administration cleared the peptide for the reduction of excess abdominal fat in adults with HIV infection and associated lipodystrophy. The decision rested mainly on two randomized phase 3 trials that enrolled roughly eight hundred patients and ran for twenty-six weeks. Participants receiving active drug showed substantially greater declines in visceral adipose tissue than those receiving placebo, while total body weight changed comparatively little. A reformulated presentation was later approved, and the product has remained a niche therapy rather than a general weight-loss agent.
Quantitation of the peptide relies mainly on reversed-phase high-performance liquid chromatography with ultraviolet detection, typically at 214 nanometers, where the peptide bond absorbs. Identity is confirmed by mass spectrometry, most often electrospray ionization coupled to liquid chromatography, and by peptide mapping after enzymatic digestion. Because related impurities differ only slightly in sequence or modification, method development emphasizes resolution rather than speed. Purity is usually reported as a percentage of the main peak area, with individual impurities listed separately when they exceed a defined reporting threshold.
Stability testing examines how the molecule changes under controlled stress. Thermal stress, light exposure, and extremes of pH are applied separately so that each degradation route can be attributed to a specific cause. The main observed changes are oxidation, deamidation, and aggregation into dimers or higher-order species. Accelerated studies at elevated temperature are used to estimate behavior over longer periods, though such extrapolation carries uncertainty. For a lyophilized powder, residual moisture and the choice of bulking agent strongly influence how quickly these changes appear.
Practical handling centers on limiting moisture, oxygen, and temperature excursions. Lyophilized material is generally held at or below minus twenty degrees Celsius, protected from light and kept sealed until use. Once reconstituted, solutions are typically kept cold and used within a short window because hydrolysis and microbial growth both accelerate in liquid form. Repeated freeze-thaw cycles are avoided, since they promote aggregation. Vial contents should be inspected for particulates and clarity before analysis, and working aliquots are prepared to reduce the number of times the stock is opened.

==== Countercurrent flow examples ==== In a countercurrent heat exchanger, the hot fluid becomes cold, and the cold fluid becomes hot. In this example, hot water at 60 °C (140 °F) enters the top pipe. It warms water in the bottom pipe which has been warmed up along the way, to almost 60 °C (140 °F). A minute but existing heat difference still exists, and a small amount of heat is transferred, so that the water leaving the bottom pipe is at close to 60 °C (140 °F). Because the hot input is at its maximum temperature of 60 °C (140 °F), and the exiting water at the bottom pipe is nearly at that temperature but not quite, the water in the top pipe can warm the one in the bottom pipe to nearly its own temperature. At the cold end—the water exit from the top pipe, because the cold water entering the bottom pipe is still cold at 20 °C (68 °F), it can extract the last of the heat from the now-cooled hot water in the top pipe, bringing its temperature down nearly to the level of the cold input fluid (21 °C (70 °F)). The result is that the top pipe which received hot water, now has cold water leaving it at 20 °C (68 °F), while the bottom pipe which received cold water, is now emitting hot water at close to 60 °C (140 °F). In effect, most of the heat was transferred.
Random freeze: the genetic code was randomly created. For example, early tRNA-like ribozymes may have had different affinities for amino acids, with codons emerging from another part of the ribozyme that exhibited random variability. Once enough peptides were coded for, any major random change in the genetic code would have been lethal; hence it became "frozen". Stereochemical affinity: the genetic code is a result of a high affinity between each amino acid and its codon or anti-codon; the latter option implies that pre-tRNA molecules matched their corresponding amino acids by this affinity. Later during evolution, this matching was gradually replaced with matching by aminoacyl-tRNA synthetases. Optimality: the genetic code continued to evolve after its initial creation, so that the current code maximizes some fitness function, usually some kind of error minimization. Hypotheses have addressed a variety of scenarios:

=== Electronic Submissions Gateway (ESG) === To standardize, automate and streamline the flow of regulatory data, FDA introduced an Electronic Submissions Gateway (ESG) in 2006. This gateway allows reporting organizations to send regulatory submissions to different centers over the internet, packaged in a center-specific format and enveloped as a GNU-compatible .tar.gz file, through either a FDA-specific WebTrader application or via a more generic B2B communication protocol called AS2 (Applicability Statement 2).
=== Production === Polystyrene is an addition polymer that results when styrene monomers polymerize (interconnect). This can be started by applying heat or benzoyl peroxide to styrene as initiators. As a liquid or a gas, pure styrene will polymerise spontaneously to polystyrene, without the need of external initiators. In the polymerization, the carbon–carbon π bond of the vinyl group is broken, and a new carbon–carbon σ bond is formed, attaching to the carbon of another styrene monomer to the chain. Since only one kind of monomer is used in its preparation, it is a homopolymer. The newly formed σ bond is stronger than the π bond that was broken, thus it is difficult to depolymerize polystyrene. About a few thousand monomers typically comprise a chain of polystyrene, giving a molar mass of 100,000–400,000 g/mol.

=== Pharmacokinetics === In non-human primates, the plasma half-life of the prodrug is 20 minutes, with the main metabolite being the nucleoside, GS-441524. Two hours post injection, the main metabolite GS-441524 is present at micromolar concentrations, whilst intact Remdesivir is no longer detectable. Because of this rapid extracellular conversion to the nucleoside GS-441524, some researchers have questioned whether the active nucleotide triphosphate is truly derived from remdesivir pro-drug removal or whether it occurs by GS-441524 phosphorylation, and whether direct administration of GS-441524 would constitute a cheaper and easier to administer COVID‑19 drug compared to remdesivir. The activated nucleotide triphosphate form has sustained intracellular levels in PBMC and presumably in other cells as well.
=== Vitriols === The study of vitriols (hydrated sulfates of various metals forming glassy minerals from which sulfuric acid can be derived) began in ancient times. Sumerians had a list of types of vitriol that they classified according to the substances' color. Some of the earliest discussions on the origin and properties of vitriol is in the works of the Greek physician Dioscorides (first century AD) and the Roman naturalist Pliny the Elder (23–79 AD). Galen also discussed its medical use. Metallurgical uses for vitriolic substances were recorded in the Hellenistic alchemical works of Zosimos of Panopolis, in the treatise Phisica et Mystica, and the Leyden papyrus X. Medieval Islamic alchemists like the Jabirian authors (those writing under the name of Jabir ibn Hayyan [died c. 806 – c. 816, known in Latin as Geber]), Abu Bakr al-Razi (865–925, known in Latin as Rhazes), Ibn Sina (980–1037, known in Latin as Avicenna), and Muhammad ibn Ibrahim al-Watwat (1234–1318) included vitriol in their mineral classification lists.

In vascular plants, the xylem and phloem are the conductive tissues that transport resources between shoots and roots. Roots are often adapted to store food such as sugars or starch, as in sugar beets and carrots. Stems mainly provide support to the leaves and reproductive structures, but can store water in succulent plants such as cacti, food as in potato tubers, or reproduce vegetatively as in the stolons of strawberry plants or in the process of layering. Leaves gather sunlight and carry out photosynthesis. Large, flat, flexible, green leaves are called foliage leaves. Gymnosperms, such as conifers, cycads, Ginkgo, and gnetophytes are seed-producing plants with open seeds. Angiosperms are seed-producing plants that produce flowers and have enclosed seeds. Woody plants, such as azaleas and oaks, undergo a secondary growth phase resulting in two additional types of tissues: wood (secondary xylem) and bark (secondary phloem and cork). All gymnosperms and many angiosperms are woody plants. Some plants reproduce sexually, some asexually, and some via both means. Although reference to major morphological categories such as root, stem, leaf, and trichome are useful, one has to keep in mind that these categories are linked through intermediate forms so that a continuum between the categories results. Furthermore, structures can be seen as processes, that is, process combinations.
He and his teammates were initially found not guilty in March 2015 by the AFL Anti-Doping Tribunal, but a guilty verdict was returned in January 2016 after an appeal by the World Anti-Doping Agency. He was suspended for two years which, with backdating, ended in November 2016; as a result, he served approximately fourteen months of his suspension and missed the entire 2016 AFL season. The suspension sparked speculation that Hooker would leave Essendon as a free agent, but he signed a new five-year contract with Essendon in April 2016, rejecting larger offers from several other clubs. On 17 August 2021, Hooker announced that he would retire at the end of this season.
== Risk factors == Although it is difficult to predict which individuals will be affected from drug-induced long QT syndrome, there are general risk factors that can be associated with the use of certain medications. Generally, as the dose of a drug increases, the risk of QT prolongation increases as well. In addition, factors such as rapid infusion, concurrent use of more than one drug known to prolong QT interval, diuretic treatment, electrolyte derangements (hypokalemia, hypomagnesemia, or hypocalcemia), advanced age, bradyarrhythmias, and female sex have all been shown to be risk factors for developing drug-induced QT prolongation. TdP has been shown to occur up to three times more often in female patients compared with males, likely as a result of post-pubertal hormonal influence on cardiac ion channels. The QTc interval is longer in females, as well as having a stronger response to IKr-blocking agents. In males, the presence of testosterone upregulates IKr channels and therefore decreases QT interval. Stated otherwise, estrogens prolong the QT interval, while androgens shorten it and decrease the response to IKr-blocking agents. Structural heart disease, such as heart failure, myocardial infarction, and left ventricular hypertrophy, are also risk factors. Diuretic-induced hypokalemia and/or hypomagnesemia taken for heart failure can induce proarrthymia. The ischemia that results from myocardial infarctions also induce QT prolongation.
== Early life == Miguel A. Ondetti was born in Buenos Aires, Argentina; on May 23, 1930. Ondetti's background was Italian. His mother was an Argentine-born Italian, while his father's family had first emigrated from Italy to Paris, then to Argentina. Despite this, Ondetti considered himself a "first-generation Argentine". Ondetti's father practiced a family craft of making garden furniture from cement that imitated tree trunks. Following the mid-1930s he became a night watchman. His mother was a housewife who cared for Miguel and his only brother, who is two years older. Ondetti's first experience with chemistry was experimenting with his older brother. Miguel tried to electroplate a knife using a copper sulfate solution, only to shock himself.

No. It is a peptide that acts upstream of growth hormone release, while growth hormone is the hormone itself. The two differ in size, in receptor, and in how the body clears them.
It blocks the amino-terminal degradation step that limits native GHRH. The addition extends how long the peptide survives in plasma without removing its ability to activate the receptor.
The chain contains 44 amino acid residues. It matches the human GHRH(1-44) sequence apart from the amino-terminal modification.
It targets the growth hormone-releasing hormone receptor on pituitary somatotroph cells. Binding stimulates cyclic AMP signaling and growth hormone secretion. This is the same receptor used by endogenous GHRH.
