== Treatment == The main goal of treatment is to treat shock and preserve kidney function. Initially, this is done through the administration of generous amounts of intravenous fluids, usually isotonic saline (0.9% weight per volume sodium chloride solution). In victims of crush syndrome, it is recommended to administer intravenous fluids even before they are extracted from collapsed structures. This will ensure sufficient circulating volume to deal with the muscle cell swelling (which typically commences when blood supply is restored), and to prevent the deposition of myoglobin in the kidneys. Amounts of 6 to 12 liters over 24 hours are recommended. The rate of fluid administration may be altered to achieve a high urine output (200–300 mL/h in adults), unless there are other reasons why this might lead to complications, such as a history of heart failure. While many sources recommend additional intravenous agents to reduce damage to the kidney, most of the evidence supporting this practice comes from animal studies, and is inconsistent and conflicting. Mannitol acts by osmosis to enhance urine production and is thought to prevent myoglobin deposition in the kidney, but its efficacy has not been shown in studies, and there is a risk of worsening kidney function.

=== Towards separation === After Slovenia and Croatia declared independence from the Socialist Federal Republic of Yugoslavia in 1991, Bosnia and Herzegovina declared its sovereignty in October 1991 and organized a referendum on independence in March 1992. The decision of the Parliament of the Socialist Republic of Bosnia and Herzegovina on holding the referendum was taken after the majority of Bosnian Serb members had left the assembly in protest. These Bosnian Serb assembly members invited the Bosnian Serb population to boycott the referendum held on 29 February and 1 March 1992. The turnout in the referendum was 64-67% and the vote was 98% in favor of independence. Independence was declared on 5 March 1992 by the parliament. The referendum and the murder of two Bosnian Serb members of a wedding procession in Sarajevo the day prior to the referendum was utilized by the Bosnian Serb political leadership as a reason to start road blockades in protest. Further political and social deterioration followed, leading to the Bosnian War. The Socialist Republic of Bosnia and Herzegovina was renamed the Republic of Bosnia and Herzegovina on 8 April 1992, losing the adjective "Socialist". It began moving toward a fully capitalist economic system. The republic retained socialist realist symbols pending the end of the Yugoslav Wars. === Mechanisms of drug-induced cholestasis === Drugs may induce cholestasis by interfering with 1) hepatic transporters, 2) bile canaliculi dynamics, and/or 3) cell structure and protein localization. Hepatic transporters are essential for maintaining enterohepatic bile flow and bile acid homeostasis. Therefore, their direct inhibition by certain drugs may lead to cholestasis. Relevant transporters implicated include BSEP, MDR3, MRP2-4, and NTCP. Cholestasis can result from competitive inhibition of BSEP by several drugs, including cyclosporine A, rifampicin, nefazodone, glibenclamide, troglitazone, and bosentan. BSEP is the main transporter in hepatocytes responsible for exporting bile salts across the apical membrane into bile canaliculi. Therefore, inhibiting BSEP should cause cytotoxic bile salts to accumulate in hepatocytes, leading to liver injury and impaired bile flow. Indeed, there is a strong association between BSEP inhibition and cholestasis in humans, and BSEP inhibitors are shown to induce cholestasis in vitro. However, hepatocytes have safety mechanisms that can compensate for impaired canalicular bile efflux. In response to cholestasis, MRP3 and MRP4 on the basolateral membrane are upregulated to allow efflux of accumulated bile salts into portal blood. Similarly, MRP2 can accommodate additional bile flow across the apical membrane in cholestatic conditions. These compensatory mechanisms explain why some BSEP inhibitors do not cause cholestasis.
